Does NAD+ Therapy Actually Work?
The cellular-decline science behind NAD+ is real. What two 2026 systematic reviews actually found is that the clinical evidence for raising it back up in a healthy adult is still genuinely inconclusive — and the route Strut Health uses has even less trial data behind it than the more common oral route.

What's genuinely well-established
NAD+ (nicotinamide adenine dinucleotide) is a coenzyme present in every cell, essential for converting food into cellular energy and for DNA repair. That NAD+ levels decline measurably with age is not in dispute — it's consistently observed across human and animal studies. That part of the pitch is accurate.
What the newest reviews actually found
Two comprehensive reviews, both published in 2026, looked specifically at what happens when you raise NAD+ levels back up in humans — whether through NAD+ precursors (like NMN or nicotinamide riboside) or direct NAD+ administration.
The honest summary. Per a PRISMA-guided systematic review in a 2026 aging research journal, NAD+ supplementation reliably does one thing very well: it raises measurable NAD+ levels in blood and tissue. What it does not yet reliably show is a consistent, confirmed clinical benefit — studies assessing real-world outcomes like fitness, metabolic health, and aging markers report mixed or null findings across the field. An earlier systematic review in the American Journal of Physiology reached a similar conclusion: NAD augmentation looks like a promising avenue for treating age-related conditions, with a generally favorable safety profile, but health benefits "are yet to be established in large human studies." Some individual trials did find real, specific positive results — including improved muscle insulin sensitivity in one 10-week study of women with prediabetes, a modest blood-pressure reduction in another trial, and reduced fatigue and improved concentration reported in a third — but the data on sleep quality and general fatigue overall was characterized as too sparse and inconsistent to draw firm conclusions.
The specific gap that matters for a subcutaneous injection
This is the finding most directly relevant to Strut Health's actual product: a compounded NAD+ subcutaneous injection, not an oral supplement. The 2026 aging-research review flagged something worth knowing plainly — despite IV and intramuscular NAD+ delivery being the most commercially popular routes people pay for, there are currently no eligible outcomes trials that have tested IV or IM NAD+ specifically for anti-aging or wellness indications in humans. A subcutaneous injection isn't identical to IV or IM delivery, but it shares the same basic category — a parenteral route bypassing digestion — and it means the clinical outcomes evidence for this specific delivery method is thinner than the evidence base for oral NAD+ precursors, which is itself already inconclusive. In plain terms: the most rigorously reviewed research gap in the entire NAD+ literature sits closest to the delivery method actually being sold here.
Where this leaves an honest verdict
None of this means NAD+ therapy "doesn't work" in the sense of being fraudulent or dangerous — the safety profile across the reviewed studies has been favorable, with no consistent serious safety signals. It means the marketing framing common across the anti-aging/wellness industry (including phrases like "mental clarity & focus" that our own investigation already flagged) is running well ahead of what controlled human trials have actually confirmed. NPR's own 2026 reporting on the space summarized it similarly: NAD+ precursors are probably best understood right now as experimental adjuncts, not established therapies with proven outcomes.
NAD+ decline with age is real, and raising NAD+ levels is biologically achievable and appears generally safe in the studies reviewed. Whether doing so produces a reliable, measurable benefit for a healthy adult — more energy, sharper focus, slower aging — remains genuinely unproven, with mixed results across the human trials that exist. The evidence gap is largest for the exact delivery category (injectable, non-oral) that a compounded subcutaneous NAD+ product falls into. That's a reason to treat it as a plausible, still-being- studied option, not a reason to assume either that it definitely works or that it's a scam.
